Mental disorders are a complex public health problem with a high prevalence that significantly affects not only patients but also their families. According to WHO (2019), one in four people worldwide will experience a mental disorder in their lifetime, while in Indonesia, the prevalence of severe mental disorders reaches 7 per 1,000 households (Riskesdas, 2018). One of the main problems faced by families is internal stigma, which refers to negative perceptions, feelings of shame, or helplessness when caring for a family member with a mental disorder. Internal stigma can reduce family support in the care process, thus mental health education is required as an intervention to improve knowledge and reduce stigma. This study aimed to analyze the effect of mental health education on the knowledge of internal stigma among families of patients with mental disorders in the working area of Puskesmas Kumun, Sungai Penuh City, in 2025. This research used a quantitative method with a One Group Pretest-Posttest Design involving 16 respondents selected based on inclusion criteria. Data were collected using a knowledge questionnaire and an observation sheet on internal stigma, and analyzed using the Paired Sample T-Test. The results showed that the mean knowledge score before education was 17.13 (SD ± 2.06) and increased to 31.56 (SD ± 2.99) after education, with a mean difference of 14.44. Statistical analysis showed a t-value of -15.082 and p = 0.000 (p < 0.05), indicating a significant difference before and after the intervention. Observation results also revealed changes in stigma behavior, where high stigma decreased from 68.8% to 18.8%, while low stigma increased from 31.2% to 81.2%. In conclusion, mental health education is proven effective in improving family knowledge and reducing internal stigma toward people with mental disorders. Therefore, sustainable educational programs at primary health centers are strongly recommended to strengthen family roles in supporting patient recovery.
